CLIENT SERVICE AGREEMENT

By booking an appointment and/or receiving services from Gynzuh Ave (“Salon,” “we,” “us”), you (“Client”) acknowledge that you have read, understand, and agree to the following terms and conditions in accordance with applicable Illinois and City of Chicago laws.
APPOINTMENTS & TIMELINESS
Clients must arrive on time for scheduled appointments.
Clients arriving more than five (5) minutes late may not be serviced due to scheduling constraints.
If service is provided despite lateness, a late service fee equal to double the service cost may apply and must be paid prior to the start of service.
If service cannot be provided due to lateness, the deposit will be forfeited.
Any exception is at the sole discretion of the stylist and is not guaranteed.
APPOINTMENT DURATION
Appointment start times are fixed.
Service durations are estimates only and may vary due to hair condition, service complexity, or unforeseen circumstances.
PARKING
Parking may be available near the salon.
Clients are responsible for complying with all posted parking signs and City of Chicago regulations.
The Salon is not responsible for parking citations, towing, or vehicle damage.
COLOR & CUSTOM SERVICES
All color services and wig customizations must be scheduled at least fourteen (14) days in advance.
Requests without required notice may incur an expedited service fee.
Turnaround time for custom services is a minimum of fourteen (14) days.
Items must be dropped off during business hours and handed directly to an on-duty stylist.
The Salon is not responsible for items left unattended, outside the salon, or not properly received by staff.
HAIR PREPARATION REQUIREMENTS
Shampoo and blow-dry of natural hair are included with applicable services.
Extension washing and drying are not included and must be completed prior to arrival unless booked as an add-on.
Hair must be fully detangled and free of matting prior to arrival.
Take-down services are not included unless selected during booking.
Services may be refused if hair condition prevents safe or timely completion.
If detangling is required and the Client elects to proceed, an additional fee of $150 per hour (pro-rated) will apply.
EXTREMELY THICK HAIR
Additional fees may apply for extremely thick hair.
Clients unsure whether this applies must submit a photo prior to booking.
NO-SHOW POLICY
Failure to attend a scheduled appointment without notice may result in charging up to 100% of the service cost to the original payment method and/or card on file, as permitted under Illinois law.
Deposits are non-refundable and non-transferable.
DEPOSITS & PAYMENTS
A deposit is required to secure most appointments and is applied toward the final balance.
Some services require full payment at the time of booking.
All deposits and payments are non-refundable and non-transferable, except where prohibited by Illinois law.
In the event of government-mandated closures (including City or State orders), appointments may be rescheduled within six (6) months. No refunds will be issued.
RESCHEDULING
Reschedule requests are not guaranteed.
If approved, a rescheduling fee may apply and must be paid at the time of rebooking.
PROMOTIONS & DISCOUNTS
Promotions, coupons, and discounts cannot be combined unless expressly stated in writing.
REFUNDS
Refunds are not provided for completed services, deposits, balances, custom work, invoices, or full-payment services, except where required by Illinois law.
Any refund issued is at the sole discretion of the Salon and does not establish precedent.
SPECIAL APPOINTMENTS
Appointments outside normal business hours or days are subject to availability and additional fees.
PHOTO & VIDEO RELEASE
By booking, the Client grants Gynzuh Ave permission to photograph and/or record the Client for marketing and promotional purposes without compensation.
All images and recordings become the property of the Salon.
The Client waives any right to inspect, approve, or receive royalties related to such media, as permitted by Illinois law.
RIGHT TO REFUSE SERVICE
The Salon reserves the right to refuse or discontinue service for any reason permitted by federal, Illinois, and City of Chicago law, including safety, health, or conduct concerns.
LIMITATION OF LIABILITY
To the fullest extent permitted under Illinois law, the Salon shall not be liable for indirect, incidental, or consequential damages arising from services rendered.
GOVERNING LAW
This Agreement shall be governed by and interpreted in accordance with the laws of the State of Illinois and applicable City of Chicago ordinances.
ACKNOWLEDGMENT
By booking an appointment, the Client acknowledges understanding and acceptance of all policies outlined above.
